Student health plans required in fall ’07

Starting in the fall of 2007, full-time undergraduate students will be required to carry health insurance as a condition of enrollment.

For the first time, PLU will be offering comprehensive health insurance, and unless a waiver form providing proof of comparable insurance is filled out, all students will be automatically enrolled and billed for the plan.

The plan provides comprehensive health coverage with no deductible, and at $400 a year or about $30 a month, it compares quite favorably with private insurance. The PLU Health Clinic will be the primary provider for students, with referrals to Pierce County health providers when necessary.

The $400 will be billed as part of the fall billing process, and financial aid money may be used to help cover the cost of the premium. Students will receive a health insurance identification card shortly after the fall term begins in September 2007.

To decline the coverage, students will need to complete an online waiver form available at www.plu.edu/health. The time frame for completing the waiver form begins in April 2007 and ends at midnight on Sept. 17, 2007. No waivers will be granted after this deadline passes.

Annual premium:
$400

Benefit period:
12 months for full-time continuing undergraduate students. Benefits terminate upon part-time status or withdrawal.

Aggregate accident benefit:
$5,000. The aggregate benefit limit for injuries and accidents related to participation in intercollegiate sports is $1,000. Intercollegiate sports coverage is provided under another policy.

Aggregate Sickness Benefit:
$10,000

Inpatient hospital benefit:
100 percent for the first $500, 80 percent thereafter up to the maximum aggregate benefit.

Outpatient sickness benefit:
This covers up to $1,000 per sickness for prescriptions, emergency room, lab expenses, diagnostic x-rays, etc. Some dental expenses due to an injury are also covered. Deductible of $50 without a PLU Health Center referral.

Outpatient surgical benefit:
Up to $1,500 per illness. Treatment for bony impacted wisdom teeth or dental abscesses covered up to $100 per tooth/$400 total.

Global emergency assistance:
Provides medical emergency and some legal assistance when you are at least 100 miles from your permanent address, campus address or in another country. Services include: medical consultation, evacuation, repatriation, prescription assistance and emergency trauma counseling.
